Position Description

The Patient Care Associate is responsible for performing direct and indirect patient care tasks as delegated by the appropriate licensed nursing personnel. The primary responsibility of the Patient Care Associate is to provide basic patient cares such as bathing and hygiene, elimination and toileting, nutrition feeding and hydration, and ambulation and positioning. The Patient Care Associate supports patient safety and patient satisfaction by performing regular patient rounds on the nursing unit and effectively communicating with all team members. The Patient Care Associate demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient.

Quality & Safety - Provides quality nursing care to patients in an environment that promotes patient safety. Provides individualized attention, which encourages each patient’s ability to maintain or attain the highest practical physical, mental and psychosocial well-being.

Patient Care - Attends to the individual needs of patients, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the individuals’ care requirements. Answers patients’ call signals promptly and courteously.

Interaction - Maintains the comfort, privacy, and dignity of patients in the delivery of services to them. Interacts with patients in a manner that promotes a caring environment.

Observation & Documentation - Observes patients for changes in condition or behavior and promptly reports these changes to appropriate licensed nursing personnel. Completes Patient Care Associate records, documenting care provided or other information in keeping with organizational policies and regulatory requirements.

Ambulating - Lifts, moves and transports patients using proper body mechanics or lifting devices for accident prevention.

Rounding - Conducts purposeful rounding to include location of personal items, patient positioning, and toileting.

Intake & Output - Consistently records intake and output.

Position Requirements

Degree/Diploma:
High School Diploma

Additional Educational Requirements:
Preferred successful completion of a recognized Nurse Assistant training course and/or possesses a significant amount of patient care experience.

Certification/License Title: CPR Certification

Additional Skills & Experience Requirements:
Previous hospital or nursing home experience preferred. Previous computer experience preferred.
