Position Description

Under the direction of the Clinical Nurse I - V, Department Director, Patient Care Manager, Shift Administrator and Vice President of Patient Care, the Licensed Practical Nurse is responsible and accountable for the quality care of patients assigned by utilizing the nursing process as defined in the Missouri Nurse Practice Act. The LPN will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient. Demonstrates understanding of the unique physical and psychological needs of the pediatric, adolescent, adult and geriatric patients and possesses the ability to assess data reflective of the patient's status and interpret the appropriate information necessary to provide quality care.

Assessment - Nursing Process (Assessment). Accurately and thoroughly completes initial assessments and reports data to nursing and/or staff. Includes input from patient/family in initial assessment and in determining priority problems. Always makes certain that patient's assessment (within assignment) is completed within time frame. Consistently provides feedback/reports of admission data to nursing staff and physicians.

Plan of Care - Nursing Process (Plan of Care). Initiates nursing and physician orders/interventions personally or by consulting with the other appropriate health care members. The LPN assists APN/RN in individualizing the care provided to meet the needs of each patient. Includes patient/family in developing and implementing the plan of care.

Implementation of Safe/Appropriate Care - Nursing Process (Implementation of Safe/Appropriate Care). Provides direct patient care within the interpreted guidelines of the Nurse Practice Act of Missouri. Participates in clarifying medical information to patient, family and staff. Interprets, transcribes, and implements physician orders accurately. Adheres to Medical Center policies and procedures during medication administration (effect/reaction), order transcription, IV therapy and all other therapeutic modalities. Provides intense emotional and psychological support for patients/family. Participates in health care teaching of patients/family utilizing appropriate teaching standard, evaluating its effectiveness. Performs only those technical skills and procedures within the scope of the LPN's training/ education and ability under the direct supervision of the RN, APN and/or physician in accordance with the Medical Center's policies and procedures.

Documentation - Nursing Process (Documentation). Documents results of care and patient's progress. Documentation reflects implementation and evaluation of nursing/physician orders and patients' progress toward outcome. Documents continuous care has been established and progress has been noted. Seeks consultation as needed from other healthcare providers. Consistently documents patient education.

Acknowledges APN/Physician Plan of Care and Practices in a Collaborative Relationship - Acknowledges APN/Physician Plan of Care and Practices in a Collaborative Relationship. Consistently assists APN/physician with examination of patient. Communicates patient's condition and progress, collaborating on the plan of care. Organized and prepared when seeking APN/physician input via phone or in person, i.e., chart available, questions organized, etc. Provides guidance and uses proper discretion when notifying APN/physician(s) of patient condition change. Consistently assures that other team members are informed of APN/physician's plan of care.

Informs leadership of risk management situations, conflicts, physician concerns, etc. - Informs leadership of risk management situations, conflicts, physician concerns, etc. Performs accurate documentation on above situations. Communicates identified problems with Director. Keeps Director informed of pertinent events. Reports malfunctioning equipment. Safe Working Environment: Demonstrates knowledge of emergency procedures, drills, evacuation procedures, including OSHA exposure control plan, through appropriate responses to emergency situations/drills and safe working habits. Has completed Super Safety Day within the past 12 months. Has recertified in CPR within the last two years.

Position Requirements

Educational Requirements: Graduate of an approved practical or vocational nursing program.

Certification/License Title: CPR Certification, Licensed to practice in the State of Missouri or eligible for licensure while having a Missouri work permit.
