Position Description

Evaluates and recommends appropriate medication therapy to attain appropriate and efficient patient outcomes by collaborating directly with physicians, nurses, and other healthcare providers. Leads the way in the development, supply, and evaluation of overall pharmaceutical care along with quality assurance programs. Serves as a resource to pharmacy colleagues and other hospital colleagues as a drug expert. Assesses patient’s health conditions and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care while documenting where appropriate. Participates in professional development activities. Participates in drug cost reduction programs.

Skills and/or Knowledge Required: May require standing for long periods; ability to move freely around the Medical Center.

Medication Therapy Intervention and Documentation - Responds to medication therapy intervention opportunities and documentation of the intervention.

Reporting errors - Reports ADE and Medication Errors as appropriate.

Committees - Leads/participates in multi-disciplinary and Medical Staff Committees as appropriate.

Clinical Pharmacy Activities - Leads the department in clinical pharmacy activities. Reviews literature for patient safety issues and leads the department in responding to newly discovered medication problems.

Completing work - Employee completes work (under normal circumstances) and does not leave work for the next shift.

Workqueue - Monitors the Clinical Workqueue and responses to alerts and warnings in an appropriate manner.

Position Requirements

Educational Requirements: PharmD and completion of ASHP-accredited PGY1 residency or equivalent required.
Graduate of a College of Pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).

Certification/Licensure Requirements: Pharmacist licensure - MO
BCPS or other Specialty certification. Must maintain specialty certifications.
Current ACLS certification BLS certification or ability to obtain.

Skills & Experience Requirements: Hospital Clinical Pharmacy Specialist experience or 3-5 years clinical hospital setting
PGY2 or equivalent experience in field 3-5 years clinical hospital setting
