Position Description

The Hospice Nurse - LPN provides nursing care to adult and geriatric patients/clients as ordered by the physician and in compliance with applicable laws and regulations and the policies of Saint Francis Hospice. Reports to Director of Home Health and Hospice and designee.

Nursing Process - Demonstrates knowledge of age-specific factors necessary to perform safety and effective performance of duties. Recognizes patient's psychological development. Assesses safety needs based on age groups and provides safety measures based on assessment. Uses appropriate-sized equipment for size and age of patient. Identifies increased physical/emotional dependency needs and evaluates availability of support system.Accurately and thoroughly completes assessment. Included input from patient/family in initial assessment and in determining priority problems. Assessments are completed within time frame. Regularly re-evaluating patient/client needs. Uses supplies and equipment effectively and efficiently. Observes the patient/client for changes in condition, discusses changes with the patient/client or family/caregiver and reports as necessary, to the RN and/or physician. Assists/instructs the patient/client with the activities of daily living and facilitates the patients/client's efforts toward self-sufficiency and independence. Teaches the patient with self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/clients and families/caregivers. The nurse demonstrates compassion and provides comfort to patients and their caregivers.

Patient Care - Includes patient/family in all aspects of patient care. Consults with other appropriate health care members. Gives organized, concise reports to oncall hospice nurse. Utilizes downtime for the promotion of patient and/or medical center goals and chart review. Routinely accomplishes work within assigned, allowable time. Organizes prioritizes care/work load. Completes all paperwork in a timely manner. Keeping patient charts current. Participates in chart review. Participates with the IDG to establish, review and update the plan of care, assist with provision or coordination of hospice care and services and making recommendations regarding policies governing the day to day provision of hospice care and services.

Implementation of safe/appropriate care - Identifies potential problems in administration of therapeutic modalities an intervenes appropriately. Excels in observation of medical effect and reaction. Provides direct patient care withing the interpreted guidelines of the Nurse Practice Act of Missouri. Participates in clarifying medical information to patient, family and staff. Excels and adheres to medical center policies and procedures during medication administration (effectiveness/reaction), order transcription, IV therapy and all other therapeutic modalities. Provides intense emotional and psychological support for patients/family and confers with ancillary services. Participates in health care teaching of patients/family utilizing appropriate teaching standard, evaluating its effectiveness. Can perform all technical skills and procedures for area with little direction. Provides and maintains a safe environment for the patient/client.

Evaluation - Always reviews and revises/updates carepath/careplans, as patient condition warrants. Utilizes appropriate referral resources. Anticipates, through collaboration with patient/family and other health team members, patient needs and caregiver needs after the patient's end of life . Maintaining compliance with Conditions of Participation and The Joint Commission.

Acknowledges the Physician's Plan of Care - Practices in a collaborative relationship with the physician. Established professional rapport with physicians and displays confidence with communication. Knowledgeable of the plan of care. Confirms that medical and nursing plans are in concert. Communicates patient's condition and progress, collaborating on the plan of care. Organized and prepared when seeking physician's input via phone. Seeks guidance and uses proper discretion when notifying physician of patient's condition changes. Performs within the limits of preparation and experience.

Documentation - Documents results of care and patient's progress. Documentation reflects implementation and evaluation of nursing/physician orders. Documents, appropriately, each nursing visit, reflecting the plan of care. Seeks consultation as needed from other healthcare providers. Consistently documents patient education. Documentation results of care and progress so that the Patient Care notes reflect implementation and evaluation of nursing and physician orders and any needed follow-up. Documents per visit, reflecting the plan of care which is inclusive of progress toward outcomes. Documents, on a continuous basis, the revisions in the care plan have been made and problems are resolved or the problems continue. Evaluating patient's progress toward goals and revising nursing plan of care, reflecting interdisciplinary teams plans of care. documents, on a ongoing basis, progress toward goals. Participates in IDG planning in collaboration with patient, family and other health team members. Evaluates and documents, upon discharge, outcome of established goals. Documents interventions for goals not achieved. Documentation is completed accurately and timely.

Prioritization - Manages Assigned patient Care by appropriate prioritizing and delegating after an assessment has been done on the needs of the patient. Keeps supervisor informed of pertinent events. Reports malfunctioning of equipment. Informs leadership of risk management situations, conflicts, physician concerns. etc. Performs accurate documentation on above situations. Communicated identified problems with patient care to manager. If standard not being met, constructively addresses issue with team member involved. Serves as resource to family and physician. Participates in IDG team as assigned. Utilizing professional demeanor in all verbal communications.

Position Requirements

Educational Requirements: Is a graduate of an accredited school of nursing. Is currently licensed as a LPN through the State Board of Nursing in Missouri or eligible for licensure.

Certification/License Title: CPR Certification

Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
Must register with the Family Care Safety Registry (FCSR) and complete the FCSR background check.

Additional Skills & Experience Requirements: Minimum of 1 year experience as a licensed nurse required; 1 year hospice nursing experience preferred. Med-surg experience, emergency , critical care or Home Health experience helpful. Complies with accepted professional standards and principles. Has satisfactory references from nursing school, previous or current employer (s) and or professional peers. Has excellent observation skills, good nursing judgement, good oral, written communication skills and good organizational skills. Is self-directed with the ability to work with little supervision. Is flexible and cooperative in fulfilling role obligations.  LI-NH#1
